How Mental Health Services are delivered in Conwy

Services to individuals with mental health problems are currently provided via a partnership arrangement with BCUHB and Denbighshire Social Services.

Primary Care

Services for many people with mental health problems are dealt with via our Primary Care Team who, following assessment, can offer advice, support and talking therapies.

Secondary Care

Our Secondary Care Teams are located within the two Conwy Community Mental Health Teams.  They comprise of Community Psychiatric Nurses, Social Workers, Psychologists, Occupational Therapists and Psychiatrists. Initially you will be assessed by a member of the team who may be able to give advice or help to resolve your problems directly with you. If it is felt that you need a key worker, you will be allocated to a member of the team who will agree and draw up a care plan with you identifying the nature of the help that is to be provided to support you.

Secondary Support Services

Your key worker may determine that you need a period of support at home or in the community from a support worker to help overcome your problems and enable you to resume your normal daily activities.
